Rosacea or Acne? Here's How to Tell the Difference (And Why It Matters)

By Maedeh Samimi, Licensed Aesthetician | Certified Acne Expert | Urban Skin Care Clinic, Roswell, GA

You wake up, look in the mirror, and see redness, bumps, and breakouts staring back at you. You've been treating it like acne for months — maybe years — and it's just. not. clearing.

Here's something I see in my treatment room all the time: rosacea and acne are two completely different skin conditions, but they're frequently mistaken for each other. And when you treat one as if it's the other, you can actually make things worse.

Let's break it down — clearly and without the guesswork.

What Is Acne?

Acne is a condition involving clogged pores. Excess oil, dead skin cells, and acne-causing bacteria (specifically C. acnes) get trapped inside the follicle, causing whiteheads, blackheads, papules, pustules, cysts, and nodules.

It can appear anywhere on the face, back, chest, or shoulders. It's often linked to hormones, diet, lifestyle habits, and the products you're using — all of which we map in detail at Urban Skin Care Clinic.

Common acne signs:

  • Blackheads and whiteheads (comedones)
  • Cysts or nodules that feel deep and painful
  • Breakouts that cycle with hormones
  • Oily or combination skin texture
  • Can appear across the entire face, back, or chest

What Is Rosacea?

Rosacea is a chronic inflammatory skin condition — not a pore or bacteria problem. It causes persistent redness, visible blood vessels (called telangiectasia), and sometimes bumps that look a lot like pimples. But the root cause is vascular and inflammatory, not bacterial.

It most commonly appears in the central face: cheeks, nose, chin, and forehead. It's more common in fair-skinned individuals and tends to run in families, but it can affect anyone.

Common rosacea signs:

  • Persistent facial redness or flushing (especially cheeks and nose)
  • Visible small blood vessels or a web-like redness under the skin
  • Burning or stinging sensation, especially after heat, sun, or spicy food
  • Bumps that resemble pimples but without blackheads or whiteheads
  • Skin that feels sensitive, reactive, or easily irritated
  • Flares triggered by heat, alcohol, stress, or sun exposure

Why Getting This Right Is So Important

This is where it gets critical: many acne-fighting ingredients can severely irritate rosacea skin.

Products with benzoyl peroxide, salicylic acid, retinoids, or exfoliating acids are staples in acne treatment — but for someone with rosacea, these can trigger intense burning, increased redness, and worsening flares. Using the wrong approach doesn't just fail to help; it can damage your skin barrier and set your progress back significantly.

On the flip side, rosacea-calming products — focused on reducing redness and strengthening the barrier — won't address the comedones, bacteria, or hormonal triggers at the root of true acne.

This is exactly why a proper skin assessment matters before any protocol begins.

Can You Have Both?

Yes — and it's more common than you might think. Some clients come in with acne and rosacea simultaneously, which requires a carefully layered approach. We address each component strategically, making sure we're not aggravating one condition while treating the other.

What I Do Differently

At Urban Skin Care Clinic, I don't guess. During your Acne Mapping Consult , I photograph and grade your skin, review your full history, and take time to identify what's actually happening beneath the surface. Is it comedonal? Inflammatory? Hormonal? Rosacea-type? A combination?

From there, you get a customized protocol — not a generic routine pulled from a shelf — with a clear timeline, honest expectations, and home-care recommendations that make sense for your skin specifically. Because your skin is unique. So is your treatment plan.
